Tooth Worms? The History of Cavities and Tooth Fillings

Scientists have discovered tooth decay in specimens that are more than 15,000 years old. The ancients once thought that cavities were caused by something called “tooth worms” … Eew! They didn’t exist, of course, but how else could humans explain the holes that cavities make in teeth?

The appearance of cavities on a widespread basis is often traced to the rise of farming. The new diet filled with grains and carbs made our mouths a haven for cavity-causing bacteria. As we added more sugar to our diets, our teeth got worse.

The “tooth worm” idea didn’t completely disappear until the 1700s when scientists finally began to understand the process of dental caries. Once that part of the puzzle was solved, they began focusing on filling existing cavities and preventing new ones.

Dental Fillings Come of Age

Many different materials, including beeswax, cork, aluminum, tin, and even asbestos, have been used to fill the holes caused by dental decay. Sometime in the mid-1800s, however, dentists began to use metal fillings such as gold, platinum, silver and lead amalgams.

The amalgam we use today is mixed from liquid mercury, silver, tin, copper, zinc, and other metals, but some patients still like the look of a gold filling. Newer options include composite-resin fillings, which are made from a tooth-colored mixture of plastic resin and finely ground glass-like or quartz particles that form a durable and discreet filling. Porcelain or ceramic fillings are natural in color, but more resistant to staining.

Can You Repair Your Tooth Enamel?

There are lots of ads out there for toothpastes that claim to repair damaged tooth enamel.

Can you treat cavities and tooth decay at home? Well, mostly, no, you can’t.

Can you strengthen your enamel at home? Very possibly—in some circumstances. Let us explain!

Cavities and tooth decay start forming when the enamel on the tooth’s surface breaks down. To discover what causes this breakdown, we need to see how chemistry works with our biology.

Tooth enamel is mainly made from calcium and phosphate ions. These minerals combine to form hydroxyapatite, crystals which make up around 95% of our enamel. Hydroxyapatite crystals are so strong that tooth enamel is the hardest part of our bodies. What can weaken a substance this strong?

Acids. Acidic foods and drinks, as well as acids created by the bacteria in plaque, strip away calcium and phosphate ions in enamel, weakening the surface of the tooth. This is a process called demineralization, and it’s the first stage of tooth decay. Left alone, weak spots will become bigger and deeper until they form cavities.

And tooth enamel, unlike the rest of your body, isn’t living tissue. It can’t regenerate. Once bacteria and acids have created a deep enough cavity, only Dr. Kerry Olszewski can repair it by removing decay and filling the tooth.

Wait, this sounds a lot more like “No, you can’t repair your enamel” and much less like “It’s possible to strengthen your enamel.” But we’re not through!

Demineralization doesn’t equal cavities—yet. Careful attention to your habits and your diet can make a difference in whether your enamel continues to weaken or becomes stronger.

Our body’s first defense against demineralization is saliva. Calcium and phosphate ions in saliva bathe the teeth throughout the day, restoring the minerals which have been lost. This is called remineralization. Saliva also helps neutralize acids from the foods we eat. But with a diet heavy in acids, or a lot of plaque buildup, saliva just can’t keep up with the damage.

That’s where “enamel-repair” toothpastes come in. Toothpastes are available that contain hydroxyapatite to restore calcium and phosphates to weakened enamel. But for many of the most common enamel-repair toothpastes, the not-so-secret secret to their effectiveness is fluoride.

Dentists recommend fluoride toothpastes for several very good reasons. Fluoride is attracted to the minerals in tooth enamel and bonds with them. Once bonded, fluoride attracts the calcium and phosphate ions in saliva, helping restore lost minerals to the enamel. Even better, when fluoride bonds with the calcium and phosphate in our enamel, fluorapatite is created. This is a crystal even stronger and more acid-resistant than hydroxyapatite.

By careful brushing and flossing to remove plaque, by watching the acids in your diet, by making sure you’re properly hydrated, and by using fluoride toothpaste, you can both reduce the risk of demineralization and help restore weak spots in your tooth enamel.

So, can enamel-repair toothpastes effectively repair your teeth? Yes, they can be effective—if demineralization is in its early stages and if you make them a regular part of your daily dental routine.

Tips for Cold and Flu Season

It’s peak cold and flu season, a time to be especially conscious about your health. We have some suggestions to help you navigate this contagious time of year, with bonus tips for a healthy smile.

  • Wash Up! 

Bacteria and viruses can live for hours and even days on non-porous surfaces like doorknobs, phones, and keyboards. That’s why it’s so important to wash your hands before touching your mouth, nose, and eyes, before brushing and flossing, and before you cook or eat. Washing with plain soap and water will eliminate most germs.

Bonus Tip: Keeping your fingers away from your mouth does more than prevent the transmission of bacteria and viruses. Fingernail biting can damage enamel. Prolonged thumb or finger sucking can affect tooth and jaw alignment. Delicate gum tissue can be injured or infected by broken or dirty fingernails.   • Stay Hydrated

Hydration supports healthy skin, helps regulate blood pressure and body temperature, and is vital for digestion and other bodily functions. Proper hydration is also important for a healthy immune system and healthy mucous membranes, which will help you fight off colds and flu. Talk to your doctor about your ideal amount of daily hydration, whether from water, other fluids, or water-rich foods.

Bonus Tip: We also need to be hydrated to produce healthy levels of saliva. Saliva washes away food particles and bacteria and helps neutralize enamel-eroding acidity in the mouth.

  • Eat Healthy Foods

A healthy diet is crucial for a healthy immune system, with foods such as lean proteins, fruits, vegetables, whole grains, and low-fat dairy products.

Bonus Tip: A balanced diet is good for your oral health, too! We need vitamins A and C for healthy gums and oral tissues, minerals like calcium and phosphorus to keep tooth enamel strong, and dairy products enriched with vitamin D, which enables our bodies to absorb the calcium we get in our diet.

What happens if you become ill despite your precautions? 

  • Reduce the Chance of Infecting Others

If you’re ill, you can help stop the spread of germs by limiting contact with other people, covering your mouth and nose when you cough or sneeze, washing your hands frequently, and cleaning the common surfaces in your house which harbor germs. 

Bonus Tip: Sharing isn’t caring when it comes to colds and flu. No matter how close you are to your family members or roommates, your toothbrushes shouldn’t be close at all. Let your brush air-dry in an upright position away from other brushes to avoid contamination. 

  • Be Choosy about OTC Medications

If you take sugary cough or cold medications, brush or rinse afterward. Sugar-free cough drops are better for your teeth than sweet lozenges which bathe your enamel in sugar as you let them dissolve. 

Bonus Tip: Don’t bite into cough drops! You risk chipping or cracking a tooth.

  • Keep Up with Healthy Habits

Be sure to keep hydrated, especially if you’ve been suffering from vomiting or diarrhea. Get the sleep you need to recover. Eat a well-balanced diet.

Bonus Tip: Stay on top of your dental hygiene and brush and floss as usual, with one caution. Vomiting brings up harsh stomach acids which weaken the mineral bonds keeping tooth enamel strong, so brushing right away can be abrasive. If you throw up, rinse your mouth with water and wait 30 minutes before brushing to allow your enamel to come back to full strength.

  • Stay Home When You’re Sick

You’ll get better faster with proper care, which includes getting the rest you need to recover. And you won’t be sharing germs with the people around you.

Should a quick bite between meals make us feel guilty? Or worse, make our teeth suffer a greater risk of cavities? Let’s avoid both those possibilities! We have several tooth-healthy snacking strategies designed to help you keep your enamel cavity-free while preventing snacker’s remorse.

But first, a word or two about why we may find snacking unappetizing.

  • Please, Don’t Feed the Bacteria!

The bacteria in plaque feed on sugars and leave acidic waste products behind. These acids attack enamel, dissolving the calcium and phosphorus minerals that keep it strong. Weakened enamel leaves teeth vulnerable to decay. That’s why Dr. Kerry Olszewski and our Mill Creek team recommend avoiding a steady diet of sugary treats.

Even if you’re not reaching for a soda, or grabbing a candy bar, many snack foods are filled with added sugars. Check the nutritional labels before you indulge to avoid sugar surprises.

  • Off Balance?

Bacteria plus sugar equals more acidic conditions in the mouth. This change in the oral pH balance also reduces our natural defenses against cavity-causing acids.

A well-balanced oral environment depends on saliva to keep acids in check. Saliva washes away lingering food particles after meals, and it neutralizes acidic conditions in the mouth over the course of the day.

How does snacking interfere? Immediately after eating or drinking, acids in saliva increase. It takes about 20 minutes for saliva’s neutralizing process to begin. When we snack throughout the day, saliva doesn’t have a chance to reduce acids as long or as effectively.

  • Staying Power

Sticky and chewy snacks tend to stick to the tops of molars and between the teeth. This gives bacteria more fuel and more time to attack tooth enamel. It’s not just candies that are the culprits here. Chips, bread, pizza crusts, pastries—these simple carbs stick to teeth and break down easily into sugars.

But we promised some *positive* strategic snacking ideas. Here are some ways to make sure that snacking doesn’t put you at a much greater risk for cavities.

  • Be Choosy

Avoid processed treats that are high in added sugars. If you’re craving something sweet, fresh fruits provide sweetness with vitamins included.

Crispy fruits such as apples and Bosc pears, as well as vegetables like celery and carrots, provide gentle scrubbing action to help clean teeth between brushings.

Snacking on cheese helps neutralize acids, and other dairy products are high in calcium and phosphorus, helping rebuild the minerals that acids leach out of enamel.

Eat whole grain breads, pastas, and pastries. They offer more nutrients, and don’t break down into sugars as easily.

Finally, when it’s time to indulge in a sugar-rich treat, save it for a meal. There’s a better chance that other foods will balance the acids created by sugar, and you’ll be getting the most out of saliva’s neutralizing abilities. Speaking of which,

  • Stay Neutral

When you’re craving something flavorful without giving bacteria more fuel for acid production, consider sugar-free gum. Sugar-free gum saves you from adding sugar to your diet, and it increases saliva production as you chew. If we give you the all-clear, chewing a piece or two of gum during the day can help curb your sugar cravings and protect your enamel.

Thirsty? Drink water instead of sodas or energy drinks with your snacks. Water washes away food particles, cuts down on acidity, and provides fluoride to strengthen and protect teeth.

  • To Brush or Not to Brush?

It’s always a good idea to brush after eating. But since eating sugary or acidic foods can leave enamel vulnerable to toothbrush abrasion, many dentists recommend waiting 30 minutes after you snack before brushing. Ask Dr. Kerry Olszewski what’s best for your teeth.

If you can’t brush, rinse with water after eating or drinking.
