What to Do When the Tooth Fairy Isn’t on Schedule

August 22 is National Tooth Fairy Day! If the Tooth Fairy is a treasured part of your child’s life, you’re just in time to celebrate! But what to do when the Tooth Fairy doesn’t arrive on schedule—when baby teeth stay longer than expected or are lost too soon? 

A baby’s 20 baby teeth tend to come in within a fairly predictable time frame. The bottom central incisors in the very front of your baby’s mouth typically make their appearance first, when your baby is around six to ten months old. Over the next few years, the remaining incisors, canines, and first molars arrive. Last on the scene are the second molars, which usually show up between the ages of 23-33 months. 

Just as baby teeth follow a pattern coming in, they tend to follow the same pattern falling out. The front teeth begin to wiggle and loosen around age six or seven, while the last of the baby teeth, the canines and second molars, are often lost between the ages of ten and 12. 

Baby teeth fall out as the adult teeth below them push up as they erupt. The top of the new tooth puts pressure on the root of the baby tooth, gradually dissolving it. As the root grows smaller and can’t anchor the tooth, the tooth begins to wiggle and eventually becomes loose enough to fall out. This leaves the adult tooth perfectly placed to grow into its proper position.

Sometimes, though, teeth linger far past their fall-out date. Sometimes, because of decay or trauma, they are lost much too early. In either case, Dr. Kerry Olszewski can provide treatment to protect little smiles now and to ensure that there’s space for the permanent teeth to erupt and align correctly.

Teeth Which Overstay Their Welcome

If that baby tooth never gets wiggly, the team at Mill Creek General Dentistry can help! When baby teeth stubbornly hang on, adult teeth can erupt behind them, creating a double row of teeth commonly known as “shark teeth.” These permanent teeth can become crowded or misaligned as they try to fit in any space available. Or a baby tooth can block an adult tooth from erupting at all. When that baby tooth just isn’t budging, an extraction will create space for the permanent tooth to erupt.

Extracting a baby tooth is generally a straightforward procedure because primary teeth have very small roots. Your child’s dental team at Mill Creek General Dentistry are experts in helping you prepare your child for the procedure in a gentle, reassuring, and age-appropriate way. 

Often, a local anesthetic is all that’s necessary for a simple extraction, but if you feel sedation would better fit your child’s needs, discuss sedation options with your dentist. After the extraction, you’ll be given clear information on how to deal with pain and swelling, which foods and drinks are best while the extraction site heals, and how to protect the area.

Teeth Which Exit Too Early

In the case of decay or trauma, Dr. Kerry Olszewski will do everything possible to save the tooth. When decay is so extensive that there’s not enough structure left to hold a filling or crown, or when there’s an infection in or around the tooth, or when an accident or injury has caused serious damage, extraction might be the healthiest option. Depending on your child’s age, further treatment might be needed afterward to protect future smiles. 

Besides their roles in eating and speaking, baby teeth save space for permanent teeth. Whenever a child loses a primary tooth too soon, the family dentist will be on the lookout for potential future orthodontic problems. If remaining baby teeth shift, taking up part of the empty space left behind by the lost tooth, the adult teeth below won’t have the space they need to erupt properly. Permanent teeth might come in at an awkward angle or in the wrong place. 

In this case, Dr. Kerry Olszewski might recommend a space maintainer. Space maintainers are small, custom-designed appliances which prevent the remaining baby teeth from shifting position while holding space open for the permanent tooth to arrive right on schedule and right where it belongs. 

It’s comforting to have charts which let us know when little teeth will typically come in and when they will typically fall out. But unexpected events might mean teeth overstay their welcome or depart too soon. How can parents help prevent tooth decay?

Children are born with a set of primary teeth – 20 to be exact – that help them learn to chew and speak, and develop enough space in the jaw for the permanent teeth that will appear several years later. Kids are especially susceptible to decay, which can cause pain and tooth loss – a problem that could interfere with oral development. As a parent, it is important that you take proactive steps to keep your child’s teeth as healthy as possible.

Bottles and “Sippie Cups”

One of the biggest culprits of childhood tooth decay is poor diet. This begins as early as a few months old, when children are often allowed to go to bed with bottles and “sippie cups” of milk or juice. The sugars in these beverages – even natural sugars – can steadily decay the teeth.

Dr. Kerry Olszewski and our staff suggest serving children milk and juice only at meal times, and limiting juice intake to just a few ounces per day. If your child becomes thirsty between meals or likes to go to bed with a bottle, serve water during these times.

Hygiene

As a parent, you can establish healthy dental habits long before your child’s first tooth erupts. Start by gently wiping your baby’s gums with a clean wash cloth during the first months of life. By age one, graduate to an appropriately sized toothbrush with fluoridated toothpaste, and brush at least twice a day.

Dental Visits

Dental visits should start young and continue on a regular basis throughout your child's life. Dr. Kerry Olszewski and our staff recommend parents bring their children to Mill Creek General Dentistry for the first time no later than the child’s first birthday. Whitening Teeth with Braces

Now that you are working hard to improve your dental health and appearance with your braces, it might seem like a logical time to whiten your teeth as well. But should you go ahead with home kits or a professional whitening? The answer might be yes, but not quite yet!

Toothpaste

The easiest way to whiten teeth is regular use of a whitening toothpaste. But these do not make a major difference in tooth color and may also contain abrasives which can damage ceramic brackets and make them more likely to stain. And, whether you have metal or ceramic braces, the brackets used are bonded to your teeth. Any part of your tooth covered by a bracket will not be affected by the whitening paste. Ask our office if you are thinking of using one of these products. We will be happy to recommend the best toothpastes to use while your braces are in place.

Whitening Strips and Trays

Whiteners can be applied at home with strips or tray kits. Strips are coated with a whitening gel and then pressed around your teeth. Tray kits provide a mouthguard-like appliance, which is filled with whitening gel. But neither strips nor tray solutions will whiten any area covered by brackets. When your braces come off, there might be noticeable differences in color on each tooth. Strips are difficult to apply with braces, and trays need to be custom-designed to fit your braces and make sure they don’t disturb your orthodontic work. One size most definitely does not fit all! Finally, these whitening agents can cause tooth and gum sensitivity, especially around the time of adjustments. Many manufacturers do not recommend using their products while you have braces. Please talk to us if you are thinking of using them.

Satisfying a Sweet Tooth

Time for some sweet talk! Many studies have been done to figure out why we enjoy sugar so much. Is it brain chemistry? Is it blood sugar getting a bit low? Is it our bodies craving a quick burst of energy?

It’s probably all of the above and more besides. After all, our biochemistry makes use of sugars on a cellular level. The carbohydrates in our diet break down into sugars, and these sugars are the body’s preferred source of energy.

Problems arise when we get too much of a good thing. There’s a difference between the carbs we need to fuel our bodies and the sugars we add to foods for flavor. Too many added sugars in the diet are linked to a number of medical conditions, including obesity, diabetes, heart disease, even prematurely aging skin.

And, of course, a sugar-heavy diet has a direct effect on your oral health.

There’s no way to sugar coat it. Plaque is mainly composed of bacteria, which feed on sugars. As they digest sugar, they produce acids. These acids attack our enamel, dissolving the calcium and phosphorus minerals which keep it strong. Weakened enamel leaves teeth vulnerable to decay, and decay creates cavities.

It’s natural to want a sweet treat every now and then, but without some attention, it’s easy to go overboard with added sugars and empty calories. If you’re searching for a middle way, balancing your love for sweet things with your love for cavity-free checkups, read on!

Be Choosy

  • Check and compare labels for added sugars. You’ll be surprised how many foods have a high sugar content, even such health-oriented foods as flavored yogurts, sports drinks, fruit drinks, and protein bars.
  • Consider the (sugar) source. White and brown table sugars and syrups break down easily as we’re eating them, adding empty calories which provide little nutrition, and increasing acidic conditions in the mouth.

Fruits, on the other hand, provide vitamins, minerals, antioxidants, and fiber along with their natural sugars. Switch out cookies, cakes, and pastries and their processed sugars for fruit when you crave something sweet.

  • Chocolate lovers, don’t despair! It’s true, unless you’re eating 100% cocoa, you are probably getting added sugar in your candy bar. But dark chocolate is rich in antioxidants, iron, zinc, magnesium, and other essential minerals, too. When you indulge in chocolate, give dark chocolate a try.
  • Not all candies are created equal! Soft candy bars and candies are healthier for your teeth than sticky or chewy confections, which tend to remain on your enamel for quite some time. Choose a treat that won’t stick with you.

Watch Your Timing

  • If you must snack during the day, it’s better to choose foods without a high sugar content. Bacteria use sugars to produce acids right away. Saliva can neutralize acids in the mouth, but it takes at least 20 minutes for the process to start.

More meals mean more sugar, more acid production, and more time for these acids to cause their damage. That’s why we also suggest you . . .

  • Eat your favorite dessert with a meal instead of waiting until later. You’ll be able to enjoy it even more knowing you’re limiting your exposure to harmful acids.
  • Taking your time is not a good idea when it comes to sweets. We don’t mean you should gobble your food. We do mean that taking sips of sugary beverages throughout the day, or sucking on slowly dissolving candies, gives you a lot more exposure to sugar over a longer period of time.

Small Changes Can Make a Big Difference

  • Drink water after enjoying something sweet to help wash away and dilute sugar.
  • Straws protect your teeth from a sugar-bath if you are drinking sodas, sweetened energy drinks, or other sugary beverages.
  • Sugar-free gum provides a burst of sweet flavor without added sugar. And even better? Chewing gum increases saliva, washing away food particles and acids and bathing teeth in enamel-strengthening minerals.
